早期防治MODS综合措施在治疗重症急性胰腺炎的意义

目的 探讨早期采用防治多器官功能障碍综合征(MODS)的综合措施对治疗重症急性胰腺炎及其并发症的重要性。方法 对21例诊断为重症急性胰腺炎患者，根据病因及病情个体化选择非手术或手术治疗。早期治疗主要措施除有效抑制胰腺酶的分泌，同时针对性地维护心、肺、脑、肾重要器官功能，积极促进胃肠功能的早期恢复。结果 早期手术6例，延期手术4例，治愈9例；平均住院日41．3天。非手术11例，全部治愈；平均住院日20．5天。死亡率4．8％。结论 早期防治MODS综合措施对重症急性胰腺炎的预后具有明显改善作用。

The significance of comprehensive measures preventing MODS in the earlystage of severe acute pancreatitis

Objective To study the significance of comprehensive measures preventing MODS in the early stage of severe acute pancreatitis.Methods As for the treatment of the twenty-one patients with severe acute pancreatitis, whether employing operation method or not all depended on the individual's pathogeny and state of an illness. The main comprehensive measures in early stage were to inhibit the secretion of pancreatin in effect, aim to maintain the functions of the patient's apparatuses such as the heart, the lung, the brain and the kidney, and accelerate the recovery of gastrointestinal function as well.Results Six patients were treated operation method in early stage and four patients in late. Among them nine patients were cured. The average days in hospital were 41.3. All the eleven patients without operation method had recovered. The average days were 20.5. Mortality was 4.8%.Conclusions The prognosis for severe acute pancreatitis was improved by comprehensive measures preventing MODS in the early stage.
