彩色多普勒检测子宫内膜癌血流及其与MVD和VEGF表达的研究

摘    要:目的 :探讨彩色多普勒检测子宫内膜癌血流及其与微血管密度 (MVD)和血管内皮生长因子(VEGF)表达的关系。方法 :采用经阴道彩色多普勒超声检测 46例子宫内膜癌肿瘤内血流信号 ,计测阻力指数(RI) ;采用免疫组织化学技术检测肿瘤 MVD及 VEGF表达。结果 :肿瘤内血流 RI值和 MVD与手术分期有关 ,而与组织学分级、肌层浸润深度、组织学类型无相关性。VEGF阳性表达率与组织学分级有关。RI值与 MVD呈负相关 (r=- 0 .2 87,P<0 .0 5) ;VEGF表达阳性者 MVD高于 VEGF表达阴性者。 VEGF表达阳性与表达阴性者之间 RI值无显著差异。结论 :上述三项指标可从不同角度反映子宫内膜癌的血管生成特征 ,彩色多普勒超声有助于对子宫内膜癌血管生成和预后的评价

Abstract:Objective:To evaluate the relationship between intratumural blood flow assessed by color Doppler(CD)and microvessel density (MVD)?the expression of vascular endothelial growth factor (VEGF) in endometrial cancer,and to evaluate the relevance of these three indexes to the clinicopathological features of the cancer Methods:The intratumural flow signal of endometrial cancer was detected preoperatively by color Doppler ultrasound in 46 patients and the lowest resistance index (RI) was used for analysis MVD and the expression of VEGF of excised tumor were assessed immunohistochemically Results:There were no correlations between RI,MVD and histologic grading,depth of myometrial invasion and histologic type;however,these two indexes were significantly correlated with surgical staging,and a significant correlation was found between the expression of VEGF and histologic grading There was a negative linear correlation between MVD and RI ( r =-0 287, P <0 05) The MVD of VEGF positive group was higher than that of VEGF negative group No statistically significant difference could be found in RI values in tumors with positive expression of VEGF compared with tumors with negative expression of VEGF Conclusions:These results suggest that above three parameters be useful in evaluating the angiogensis and tumor prognosis of endometrial cancer
