经阴道彩色多普勒超声检查对宫颈管癌的诊断价值

目的探讨经阴道彩色多普勒超声检查对内生型和颈管型宫颈癌的诊断价值。方法应用经阴道彩色多普勒超声检查33例内生型或颈管型宫颈癌患者,其中0期4例,Ⅰ期14例,Ⅱ期12例,Ⅲ期3例,观察宫颈癌的宫颈大小、形态、内部回声、子宫体、宫旁组织、血流分布,分析宫颈管癌的声像图特征。结果超声检查显示,4例0期原位癌和3例Ⅰ期宫颈癌形态无异常表现,11例Ⅰ期宫颈癌的宫颈管回声不均匀,粘膜偏移、中断或消失,Ⅱ～Ⅲ期宫颈癌表现宫颈失去正常宫颈的形态,见不均匀回声区,3例Ⅲ期宫颈癌见宫腔积液、子宫体回声不均匀。26例宫颈癌肿块内可见到丰富的血流信号,血流信号呈高速低阻型。结论经阴道彩色多普勒检查对诊断内生型和颈管型宫颈癌有应用价值。

The Value of transvaginal color Doppler ultrasound in the diagnosis of canal Cervical carcinoma

Objective To study the clinical applications of ultrasonograhy in the diagnosis of canal cervical carcinoma and endogenetic cervical carcinoma.Methods Thirty-three cases of cervical carcinoma were examined by transvaginal color Doppler ultrasonography to observe the cervical size,shape,echo and color Doppler flow imaging.Among them,four cases were in stage 0,fourteen cases were in stageⅠ,twelve cases were in stage Ⅱ and three cases were in stage Ⅲ.Results There was little ultrasonic characteristics of four cases of stage 0 and three cases of stageⅠ.The ultrasonic characteristics of eleven cases of stageⅠ were heterogeneous echo of canal cervical carcinoma and mucosa offset,discontinuous and obliterate.The ultrasonic characteristics of cervical carcinoma of stage Ⅱ and Ⅲ were enlarged size,irregular cervical shape and cervical mass.Twenty-six cases of the tumors showed blood flow features of high velocity and low resistant.Conclusion The transvaginal ultrasonography has value in the diagnosis of canal cervical carcinoma and endogenetic cervical carcinoma.
