慢性肺心病T淋巴细胞亚群变化的初步观察

应用抗人Ｔ细胞单克隆抗体ＣＤ（ＣｌｕｓｔｅｒｓｏｆＤｉｆｆｅｒｅｎｔｉａｔｉｏｎ）系列，对慢性肺心病的急发期２０例和缓解期１２例及正常对照组１７例的Ｔ细胞亚群的变化进行观察对比。对部分病人的的血气变化及植物血凝素（ＰＨＡ）皮试与ＣＤ４／ＣＤ８比值变化的相关性进行了分析。结果表明，慢性肺心病急发期ＣＤ４细胞减少，ＣＤ４／ＣＤ８比值降低，而在缓解期Ｔ细胞亚群没有明显变化；ＣＤ４／ＣＤ８比值变化分别与动脉血氧分压（ＰａＯ２）及动脉血氧饱和度（ＳａＯ２）呈正相关；多数急发期病人（１０／１２）ＰＨＡ皮试低于正常。说明慢性肺心病在急发期存在Ｔ细胞亚群的异常和细胞免疫功能降低，这种改变可能继发于机体的缺氧状态。

CHANGES OF T-LYMPHOCYTE SUBSETS IN COR PULMONALE

The percentages of CD3,CD4,CD8 and CD4/CD8 ratio of the peripheralblood in 32 cor pulmonale patients(2o in acute stage,12 in relaxed stage)and 17 normalcontrols were studied. The correlations of CD4/CD8 ratio to blood gas analysis and PHA-skin-test were also analyzed in some of the patients.Compared with normal controls,CD4 cell percentage and CD4/CD8 ratio in the acute stage group were significantly de-creased. But there wasn' t any significant change in the relaxed stage group.The correla-tions of CD4/CD8 ratio to PaO2 and SaO2 were significantly positive. PHA-skin-testwere abnormal in most cor pulmonale patients at the acute stage(10/12).It s suggestedthat T-lymphocyte subsets are abnormal and cell-mediated immunity in th acute stageof cor pulmonale decreases.These changes may be related to hypoxidosis.
