不同内固定方法治疗股骨远端骨折的疗效分析

目的评价不同内固定材料治疗股骨远端骨折的临床疗效。方法回顾性分析1996—2007年187例股骨远端骨折手术治疗，并分析其并发症。结果128例病人获得随访，平均随访9．3个月（6～24个月）。5例发生内固定失效、钢板断裂，需要重新固定植骨。3例延迟愈合，Ⅱ期植骨。其余骨折均愈合。4例术后伤口感染，2例晚期出现肢体短缩，范围1～2．5cm。8例因骨折愈合后膝关节屈曲小于90°需要行松解手术。各内固定组优良率无显著性差异。结论手术治疗股骨远端骨折能够恢复解剖结构，从而最大限度地恢复肢体的功能。4种内固定都能起到较好治疗效果，手术适应证选择得当和内固定材料的规范操作是关键。

Analysis of Clinical Effect of Distal Femoral Fractures with Different Internal Fixation Methods

Objective To evaluate the clinical effect of different internal fixation methods for distal femoral fractures. Methods 187 cases of distal femoral fractures treated with different methods in our hospital from 1996 to 2007 were studied retrospectively. And analysed the complications. Results 128 cases were followed up for 9.3 months（from 6 to 24 months）,internal fLxations of 5 cases failed, and 3 cases received second bone graft for delayed union. Infection occurred in 4 cases, late limb shortening（ 1 - 2.5cm） in 2 cases, and 8 cases underwent releasing operations because the flexion of the knee was less than 90. There was no significant difference among these groups. Conclusion An operation can get good results in treatment of the distal femoral fractures. Distal femoral fractures can be managed effectively by any one of the four kinds of internal fixation,the key ooints of success are seleeting suitable indications for onerations and aecting on operation principle.
