彩色多普勒超声对锁骨下动脉盗血综合征血流动力学的研究

目的探讨锁骨下动脉盗血患者椎动脉的血流动力学与锁骨下动脉狭窄程度的关系。方法经临床及相关检查证实的30例锁骨下动脉盗血患者,并按照所测的锁骨下动脉内径分为3组,分析其椎动脉的收缩期的血流速度及频谱特征,再对上述各组间指标进行比较。结果在不同狭窄程度下,椎动脉血流频谱会发生相应变化。在重度及闭塞的情况下,患侧椎动脉血流速度较轻中度狭窄时减慢明显(P0.05);而健侧椎动脉血流速度较轻中度狭窄时增快明显(P0.05)。结论可以通过椎动脉的血流动力学变化及频谱特点分析、判断锁骨下动脉的狭窄程度。

Investigation of Hemodynamics in Subclavian Steal Syndrome by Color Doppler Ultrasonography

Objective To investigate the relationship between vertebral arterial hemodynamics in subclavian steal syndrome （SSS） and stenosis degree of subclavical artery. Methods Thirty cases of SSS confirmed by clinical and coherence examinations were divided into three groups and their vertebral arterial systolic velocity and the features of Doppler spectrum were analyzed. Results （ 1 ） The relative changes of the vertebral artery Doppler spectrum occurred in different degree of stenosis. （2）Compared with light-to-moderate stenosis, the affected vertebral arterial systolic velocity was obviously decreased in severe vertebral arterial stenosis and occlusion （P〈0.05）, while the non-affected vertebral arterial systolic velocity was obviously increased in severe vertebral arterial stenosis and occlusion （P〈0.05）. Conclusion By analysis of the changes in the vertebral artery hemodynamics and the features of Doppler spectrum, we can judge degree of the subclavical artery stenosis.
