胎心监护预测胎儿宫内窘迫的临床应用

摘    要:目的探讨胎心监测联合妊娠特异性β1糖蛋白(PSβ1G)和乳酸测定在预测胎儿宫内窘迫的临床应用。方法对产前、产时使用胎儿电子监护仪经腹壁监测胎心率和宫缩的孕妇资料进行回顾性分析,选择胎心监护异常曲线的孕妇160例作为研究对象,分为两组:PSβ1G正常组为对照组,72例;PSβ1G异常组为观察组,88例。比较两组孕妇的羊水和母血中的乳酸水平和产后异常因素。结果观察组的羊水乳酸水平高于对照组(P0.05),母血乳酸与对照组无明显差异(P0.05);观察组新生儿状况、羊水污染程度及产后异常因素均明显多于对照组(P0.05)。结论胎心监测联合PSβ1G、羊水乳酸检测能较为准确地预测胎儿宫内窘迫。

Abstract:Objective To investigate the clinical use of fetal heart monitoring combined with pregnancy-specific β 1 glycopretein and lactic acid to predict fetal distress. Methods The pregnant data of monitoring fetal heart rate and uterine contraction with electronic fetal monitor were analyzed retrospectively, and the 160 cases with abnormal fetal heart monitoring curve were selected as objects of study. They were divided into two groups: 72 cases of normal PS β1G group as control group and 88 cases of abnormal PSβ1G group as observation group. We compared the lactic acid in the amniotic fluid and maternal blood with postpartum abnormal factors between the two groups. Results The lactic acid in amniotic fluid of the observation group was much higher than that of the control group (P〈0.05). There was no difference in lactic acid in maternal blood between the two groups(P〉0.05). The postpartum abnormal factors of the observation group were more than those of the control group (P〈0.05). Conclusion The fetal heart monitoring combined with PSβ1G and lactic acid in the amniotic fluid can predict fetal distress more accurately.
