系统家庭合并舍曲林治疗抑郁症的临床研究

目的 比较系统家庭合并舍曲林治疗与单用舍曲林治疗抑郁症的临床疗效.方法 将76例抑郁症患者随机分为系统家庭合并舍曲林治疗组(联合组)和单用舍曲林治疗组(单药组),每组38例.采用汉密尔顿抑郁量表(HAMD-17)、大体评定量表(GAS)评定疗效,同时对治疗中出现的不良反应,用症状量表(TESS)进行评定,比较2组疗效、不良反应发生率及2年内复发率.结果 2组在治疗2、4周后的HAMD总分及减分率、GAS评分,较治疗前均有明显降低(P＜0.01),而2组间比较差异无统计学意义(P＞0.05);在治疗8、12周后,联合组的HAMD总分减分率、GAS评分较单药组差异有统计学意义(P＜0.05);联合组的2年内复发率20%(7/35)较单药组复发率36%(13/36)差异有统计学意义(P＜0.05).结论 系统家庭合并舍曲林治疗较单用舍曲林治疗抑郁症的远期疗效有了进一步的提高,并降低了复发率.

Systemic Family Therapy Combined with Sertraline Treatment for Depression

Objective To investigate the clinical efficacy of systemic family therapy combined with sertraline in the treatment of major depression.Methods Seventy-six patients were randomly assigned to treatment with sertraline alone（sertraline group,n=38） or in combination with systemic family therapy（therapy group,n=38）.Hamilton Depression Rating Scale（HAMD-17）,Gross Rating Scale（GAS） and Treatment Emergent Symptom Scale（TESS） were used to evaluate the effectiveness,adverse reactions and recurrence rate within 2 years.Results HAMD total scores,reduction rate and GAS scores significantly decreased at 2 and 4 weeks after treatment in both groups（P0.01）,but no obvious differences were found between the two groups（P0.05）.At 8 and 12 weeks after treatment,GAS scores and reduction rate of HAMD total scorers in combination group were significantly different from sertraline group（P0.05）,and the recurrence rate within 2 years in combination group（20%,7/35） was significantly lower than that in sertraline group（36%,13/36）（P0.05）.Conclusions The combination of systemic family therapy and sertraline is superior to sertraline alone for achieving long-term effectiveness and decreasing recurrence rate in the treatment of major depression.
