探讨老年高血压病患者血压昼夜节律变化与颈动脉斑块的关系

目的探讨老年高血压病患者血压昼夜节律对颈动脉粥样硬化斑块的影响。方法将130例老年原发性高血压病患者按照24h动态血压检测结果分为两组：非杓形组62例，杓形组68例；超声检测颈动脉粥样硬化斑块程度及性质，并对斑块进行分级。结果非杓形组颈动脉粥样硬化斑块检出率为93．55％，高于杓形组斑块检出率77．94％（P〈0．05）；非杓形组的斑块程度分级高于杓形组（P〈0．05）；非杓形组颈动脉斑块软斑及溃疡斑多于杓形组（P〈0．01）。结论血压昼夜节律异常是颈动脉粥样硬化斑块形成的危险因素。

Investigation of the relationship between day-night rhythm change and carotid atherosclerosis in elder patients with hypertension

Objective To investigate the relationship between day-night rhythm and carotid atherosclerosis in elder people with hypertension. Methods One hundred and thirty elder patients with essential hypertension were divided into 2 groups according to 24 h ambulatory blood pressure detection: non-dipping group(ND group, 62 cases, blood pressure-decreasing ratio at night, day-night rhythm disappeared), dipping group(D group,68 cases, blood pressure-decreasing ratio at night, day-night rhythm existed).The degree and character of carotid atherosclerosis were examined by ultrasound, and the plaques were classified. Results The detection rate of carotid atherosclerosis in ND group(93.55%) was higher than that in D group(77.94%)(P＜0.05). The grade of carotid atherosclerosis in ND group was also higher than that in D group (P＜0.05). The number of soft plaques and ulcer plaques in ND group was more than those in D group (P＜0.01).Conclusion Day-night rhythm is a dangerous factor which might lead to formation of carotid atherosclerosis .
