男性乳腺癌的15例临床分析

目的：探索男性乳腺癌早期诊断治疗的方法和方式。方法：本组行乳癌根治性切除术12例，单纯乳腺切除术2例，单纯睾丸切除术1例。术后4例接受一个疗程化疗，11例末作任何治疗。结果：术后14例得到随访，随访时间1～13年。生存5年以上5例，10年以上2例。5例术后1～3年均死于肺、骨转移，1例术后半年死于心梗，1例睾丸切除术后1年死于肺转移，3例随访不满5年。结论：男性乳腺癌的特点为老年、病程长、临床发现晚、易误诊。因肿瘤恶性程度高，腋窝淋巴转移率达80％，故预后差。因此应重视早期诊断和早期治疗。对男性乳腺癌应以根治术为主，术后加放疗、化疗等综合治疗，尤其要重视内乳区放疗。对怀疑男性乳腺癌患者应术中冰冻，尽量作一次性根治。术后可进行免疫组化测定，以指导术后治疗。

The Early Diagnosis and Treatment of Male Mammary Cancer(15 Cases)

Objective: The purpose of this study was to discuss the ear;y diagnosis and management of male mammary can-cer. Materials and Methods: 12 cases were operated by radical mastectomy' 4 cases received one course of chemo therapeuticmethod after operation and the others without any therapy. Results: 5 of 14 cases who were followed up from one year to thir-teen years with more than five years survival and 2 cases up to more' than ten years. After operation, 5 cases died of lung andbone metastasis within one to three years, 1 case passed away because of cardiac infarction at the sixth month, 1 case with tes-tectomy died of lung metastasis within one year. Conclusion: The characteristics of male breast carcinoma showed themetastatic rate of lymph nodes at homolateral armpit reached 80 % with high malignancy in old age. Long course late stadiumLed to poor prognosis. For this reason, early diagnosis and treatment are necessary. The comprehensive methods for this dis-ease should be carried out, including radical mastectomy, chemotherapy, Radiotherapy and endocrinotherapy. Radical mastec-tomy should be the first choice.
