颈丛麻醉复合舒芬太尼在甲状腺手术中的应用

目的对比单纯颈丛阻滞麻醉和双侧颈丛阻滞复合舒芬太尼两种麻醉方式在甲状腺手术中的效果。方法甲状腺手术患者130例，将其随机分为观察组和对照组各65例，对照组仅采用双侧颈丛阻滞麻醉，观察组在对照组麻醉的基础上静脉注射舒芬太尼0．1μg/kg。观察麻醉满意度，分别记录阻滞前和阻滞后的收缩压（SBP）、舒张压（DBP）、心率（HR）、血氧饱和度（SpO2）的变化。结果两组患者麻醉前SBP、DBP、HR以及SpO，差异均无统计学意义（P〉0．05）；麻醉后两组SBP、DBP、HR相比差异均有统计学意义（P〈0．05）；麻醉前、后SpO2的变化两组间差异无统计学意义（P〉0．05）；麻醉效果上两组差异有统计学意义（P〈0。05）；术中并发症声音嘶哑及呼吸困难的出现率两组间比较，观察组明显低于对照组（P〈0．05）。结论颈丛麻醉复合舒芬太尼用于甲状腺疾病手术，患者满意度高，且不良反应轻微，值得临床推广应用。

Application of cervical plexus anesthesia combined with sufentanil in thyroid surgery

Objective To compare the effectiveness of simple cervical plexus anesthesia and bi- lateral cervical plexus anesthesia combined with sufentanil in thyroid surgery. Methods One hundred thirty cases of thyroid surgeries were enrolled and randomly divided into the observation group and the con- trol group ,65 cases in each. The control group only received bilateral cervical plexus block. On the basis of anesthesia in the control group, intravenous sufentanil（0.1 Ixg/kg）was injected for the observation group. The systolic blood pressure （SBP）, diastolic blood pressure （DBP）, heart rate （HR）and blood oxygen satu- ration（ SpO2 ） change were observed for the evaluation of anesthesia. Results The SBP, DBP, HR, and SpO2 were not significantly different between the two groups before anesthesia（ P 〉 0.05 ） ; the changes of SBP, DBP and HR after anesthesia were significantly different （ P 〈 0.05 ） ; the post-anaesthetic SpO2 changes between the two groups showed no significantly difference （ P 〉 0.05 ） ; the anesthetic effects be- tween the two groups were significantly different（ P 〈 0.05 ） ; the occurrence rate of intraoperative compli- cations, such as hoarseness and dyspnea was lower in the observation group compared with that in the con- trol group（ P 〈 0.05 ）. Conclusion The application of cervical plexus anesthesia combined with sufen- tanil in thyroid surgery is satisfactory for the patients and the adverse reactions are mild ,which is worthy of spreading.
