腹腔镜手术在胃肠道急腹症中的应用价值

目的 观察腹腔镜手术在急腹症中的治疗效果.方法 回顾性分析517例腹腔镜探查手术与347例开腹手术的急腹症患者,根据病种分为阑尾炎组、胃十二指肠穿孔组、肠梗阻组及其他少见病种组,比较两种手术方式在各组病例中手术时间、胃肠功能恢复时间、治疗总费用、住院时间等方面的差异.结果 腹腔镜手术在阑尾炎组、胃十二指肠穿孔组及肠梗阻组的手术时间、胃肠功能恢复时间、治疗总费用、住院时间等方面均少于开腹组（P＜0.05）;在少见病种组,由于病例数目少,尚无法进行统计学比较,但观察数值,腹腔镜手术组小于开腹手术组.结论 在胃肠道急腹症中采用腹腔镜技术进行探查、诊断及治疗,是安全、有效的,并且具有良好的经济效益和社会效益.

Application value of laparoscopic operation in gastrointetinal acute abdomen

Objective To evaluate the effect of laparoscopic operation in acute abdomen. Meth- ods The clinical data of 517 cases of laparoscopie operation and 347 cases of exploratory laparotomy were retrospectively reviewed. According to the category of disease, the cases were divided into the group of ap- pendicitis, the group of gastroduodenal perforation, the group of intestinal obstruction and the group of other rare diseases. Differences in operation time, resumption time of gastrointestinal function, hospital stay and total medical cost were compared among the groups. Results In patients with appendicitis,gastroduodenal perforation or intestinal obstruction, the operation time, resumption time of gastrointestinal function, hospital stay and total medical cost in the laparoscopic operation group were less than that in the exploratory lapa- rotomy group. In patients with other rare diseases, the number of enrolled cases was too small to analyse （ P 〈 0.05 ）. But the observed values in the laparoscopic operation group were less than that in the explora- tory laparotomy group. Conclusion The ]aparoscopie exploration operation, diagnosis and treatment of a- cute abdomen are safe and effective, which has the advantages of economic and social benefits.
