食管癌与其它开胸术后乳糜胸的对比分析

目的探讨食管癌术后乳糜胸(食管组)与其它开胸术后乳糜胸(其它组)的临床特点。方法回顾分析2086例食管癌术后乳糜胸27例,以及1659例其他开胸术后4例乳糜胸的临床特点。结果食管癌组术后乳糜胸的发生率为1.29%,其他组为0.24%。食管癌组再手术率为96.29%,其他组为25%。本组所有病例均痊愈出院。结论食管癌术后乳糜胸的发生率、病因、临床表现、诊断及治疗与其他开胸术后乳糜胸有显著不同。电视胸腔镜胸导管结扎术值得推荐应用。

The Analysis of Clinical Features for Chylothorax Between Esophageal Operations and the Other Thoracic Operations

Objective To study the clinical features of postoperative chylothorax for esophageal cancer(PCEC) and to compare them with those for the other thoracic operations(OTO) Methods Retrospectively analyzed the clinical characteristics of 27 patients with chylothorax among 2086 cases for esophageal cancer,as well as 4 among 1659 cases for the other thoracic operations.Rusults The incidence of PCEC was 1.29% and that of OTO was 0.24%.The percentage of diagnosis confirmed within 4 postoperative days was 70.37% for PCEC,and 25% for OTO.The rate of typical chylous pleural effusion was 3.7% for PCEC,and 75% for OTO.Symptoms and signs of OTO were much milder than those of PCEC.The re-operation rate was 96.2% for PCEC,and 25% for OTO.All patients were discharged uneventfully.Conclussions The incidence,causes,clinical manifestations,diagnosis,and treatment of PCEC are different from those of OTO.
