血清肌钙蛋白Ⅰ在急性有机农药中毒早期心肌损伤的检测意义

目的探讨血清心肌肌钙蛋白I(cTnI)在急性有机农药中毒(AOPP)患者心肌损伤早期检测意义及其对心脏事件发生率的检测意义。方法将50例AOPP患者分轻度(A组),中度(B组),重度(C组),在就诊4小时(h)内,第24h分别检测cTnI,肌酸激酶同工酶(CK-MB),心电图(ECG);比较cTnI和其他项目对三组AOPP患者心肌损伤早期检出率的差异性。结果C组cTnI明显高于A,B组,有显著性差异;B组亦高于A组;有显著性差异;各组CK-MB,ECG无显著性差异(P>0.05);cTnI升高的例数显著高于其他项目(P<0.05)。结论应用血清cTnI检测方法能提高AOPP患者早期心肌损伤的检出率,其灵敏度高于CK-MB和ECG检查,并能预测心脏事件的发生。

The Diagnostic Significance of Plasma Cardiotroponin I (cTnI) in Early Cardiac Damage With Acute Organophosphorus Pesticide Poi-soning

Objective To explore the diagnostic significance of plasma cardiotroponin I (cTnI) in early cardiac damage with acute organophosphorus pesticide poisoning and it's detection effect of heart events incidece. Methods 50 cases with AOPP were divided into group A(mild),groupB(moderate),group C(severe).cTnI, CK-MB and electrocadiogram(ECG) performed within 4 hours and the twenty-four hours afer admission .Plasma cTnI was compared as single index and other index,in order to obtain the difference with early detectabie one for cardiac damage in patients with AOPP. Results The cTnI level in group C was signicance difference higher than in group B,A. The cTnI level in group B was signicance difference higher than in group A too. there was no signicance difference to CK-MB and ECG among the all groups(p>0.05).The cases with elevated cTnI were signicantly higher than other cases with abnormal index(p<0.05). Conclusion Mesuring plasma cTnI can improve the detectable rate to cardiac damage in patients with AOPP,and it's sensitivity were better than that of CK-MB and ECG. Mesuring plasma cTnI can predict heart events .
