地尔硫卓在老年原发性高血压病人手术中的应用

目的观察地尔硫卓对术中老年人原发性高血压的降压效果与安全性。方法选择需要术中降压的老年原发性高血压20例,术中根据血压需要静注地尔硫卓10 mg,记录给药后1、3、5、10、30、60 m in血压、心率,并观察心电图ST-T的变化及不良反应。结果所有病人给药后血压下降平稳,收缩压、舒张压、RPP(心率收缩压乘积)较治疗前均有显著下降(P<0.05),心率无明显变化,未发现严重不良反应,有65%的病人心电图ST段较术前有所改善。结论地尔硫卓用于老年原发性高血压病人术中降压,起效快,作用显著,降压幅度适当,安全性高,且心肌缺血明显改善。

Aplication of Diltiazem to Elderly Hypertension during Surgical Operation

Objective To observe the effect and security of diltiazem on elderly hypertension during surgical operation.Methods Twenty senile patients with primary hypertension were intravenously injected 10 mg of diltiazem during operation.Blood pressure,heart rate(HR) and electrocardiogram were monitored and side effects were recorded respectively at 1-,3-,5-,10-,30-and 60-min after injection.Results Blood pressure were decreased steadily.Systolic pressure,diastolic pressure and RPP were significantly decreased when compared with that of pre-injection(P<0.05).HR were not significantly changed,and severe side effects were not found.ST-segment was improved to some extent in 65% of the patients when compared with that of pre-operation.Conclusion Diltiazem,which ameliorates myocardial ischemia significantly,can decrease the blood pressure of those with elderly hypertension down to a proper level with a secure and rapid effect during surgical operation.
