COPD合并心力衰竭患者B型钠尿肽水平的变化

目的:探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并充血性心力衰竭(congestive heart failure,CHF)患者B型钠尿肽(B-type natriuretic peptide,BNP)水平的变化及临床意义.方法:选取慢性阻塞性肺疾病急性加重期(acute exacerbation chronic obstructive pulmonary disease,AECOPD)患者(59例)作为A组,COPD合并CHF患者(63例)作为B组；另选取健康体检正常者(30例)作为C组.采用免疫荧光快速测试法测定受试者血浆BNP水平并分析.结果:血浆BNP水平B组显著高于A组和C组(P＜0.01)；而A组显著高于C组(P＜0.01).B组患者治疗后血浆BNP水平明显下降(P＜0.01),治疗后血浆BNP水平B组略高于A组但无显著性差异(P＞0.05).结论:COPD合并CHF时BNP水平明显升高,提示BNP测定对鉴别COPD合并CHF有一定意义,同时可作为COPD合并CHF患者治疗效果的评价指标.

The changes and clinical significance of B-type brain natriuretic peptide in patients with chronic obstructive pulmonary disease with congestive heart failure

Objective: To explore the changes and clinical significance of B-type natriuretic peptide (BNP)in patients with chronic obstructive pulmonary disease(COPD)with congestive heart failure(CHF). Methods:Using the peripheral blood from 59 patients with the AECOPD, 63 COPD patients with CHF,and 30 healthy people as samples. Results: The levels of BNP in COPD patients with CHF were significantly higher than AECOPD group,healthy control group ( P < 0.01) ; AECOPD group higher than healthy control group ( P <0.01 ) . The levels of BNP in COPD patients with CHF were significantly decreased after treatment ( P <0.01) . The levels of BNP in COPD patients with CHF were slightly higher than AECOPD group after treatment ( P>0.05). Conclusion: The levels of BNP in COPD patients with CHF increased, Prompted BNP determination to identify Cardiac heart disease and pulmonary dyspnea have some significance,and can be used as COPD with CHF the judgment of the treatment effect of CHF in patients with heart failure .
