慢性肾脏病患者不同时期脂联素水平与慢性炎症反应状态及CRP关系的研究

目的：探讨慢性肾脏病（CKD）患者不同时期血清脂联素（ADP）水平的变化,了解不同时期脂联素水平变化与C反应蛋白（CRP）的关系,分析脂联素对炎症状态的影响及其临床意义。方法：慢性肾脏病患者90例,应用改良简化MDRD方程计算GFR,根据GFR分CKD1期组、CKD2期组、CKD3期组、CKD4期组、CKD5期组,分别测定血清脂联素水平、CRP,对不同时期患者间的脂联素水平进行比较及统计学分析,并对脂联素及CRP在不同时期进行相关性分析。结果：CKD患者不同时期的血清ADP水平及hs-CRP水平均升高,与对照组相比较均有显著性差异,且各组间的ADP、hs-CRP亦有显著性差异（P〈0.01）,脂联素与CRP在肾功能正常对照组无明显相关性,在CKD1~CKD5期组,均呈负相关。结论：随着肾功能的进展,脂联素水平明显升高,通过减轻炎性反应对肾脏病患者减少和防止并发症的发生具有重要意义。

Adiponectin Level in Chronic Kidney Disease(CKD) Patients of Different Periods and its Relationship with C-reactive Protein(CRP)

Objective：To investigate the level of serum adiponectin in CKD patients of different periods,to understand the relationship between adiponectin and CRP in CKD patients of different periods and to analyze its influence on inflammatory reaction and its clinical significance.Methods：According to glomerular filtration rate（GFR）,90 CKD patients were divided into five groups： 1、2、3、4、5 stages of CKD groups,Serum adiponectin and CRP of five patients were measured.Serum adiponectin level in CKD patients of different periods were compared and statistics analyzed and its relationship with CRP were analyzed.Results：Serum adiponectin and CRP of CKD patients of different periods step ed up all,and existed statistics analyze significance compare to control group;Serum adiponectin and CRP existed significant difference in every group （P〈0.01）, There were no significant correlation between adiponectin and CRP in control group with normal renal function,and there were negative correlation between adiponectin and CRP in 1、2、3、4、5 stages of CKD groups.Conclusion：With the progress of renal function,adiponectin levels were significantly elevated,by reducing the inflammatory reation,preventing and reducing complications of CKD patients has great significance.
