全反式维甲酸联合肝动脉化疗栓塞治疗大鼠移植性肝癌的实验研究

目的评价全反式维甲酸（ATRA）联合肝动脉化疗栓塞术（TACE）治疗大鼠移植性肝癌的疗效。材料和方法将27只Wistar大鼠采用移植法建立Walker-256肝癌模型,于移植术后11天后行MRI检查,测量肿瘤体积（V1）,再随机分成3组：A组：经肝动脉灌注ATRA0.1mg＋MMC0.05mg＋超液态碘油0.05ml＋0.025mg明胶海绵粉;B组：MMC0.05mg＋超液态碘油0.05ml＋0.025mg明胶海绵粉;C组：经肝动脉灌注生理盐水0.2ml。14天后再行磁共振检查以确定肿瘤体积（V2）,对三组间肿瘤生长率（V2/V1）进行比较。通过病理检查检测A、B、C三组间肿瘤大体表现及组织学表现,通过免疫组织化学方法检测von-Willebrandfactor（Ⅷ因子）和Ki67在肿瘤组织中的表达。结果27只大鼠经MRI检查证实均种植成功。A、B、C三组间肿瘤生长速率不完全相同（P〈0.01）,肿瘤大小术前分别为88.26±3.47mm3,87.91±5.63mm3,87.08±6.51mm3;术后两周分别为389.77±54.13mm3,609.05±106.91mm3,1116.64±120.96mm3,两两比较各组间均有显著性差异（P〈0.05）;A组肿瘤呈膨胀性生长,界限清楚,B组呈膨胀-浸润性生长,界限较模糊,C组标本光镜下提示肿瘤界限难以分清,呈现浸润性生长;A、B、C三组Ⅷ因子和Ki67阳性表达率均依次增高。结论全反式维甲酸（ATRA）联合肝动脉化疗栓塞术（TACE）对大鼠移植性肝癌具有显著的生长抑制作用,其疗效优于单纯的TACE。
