NIPPV治疗早产儿原发性呼吸暂停反复发作疗效观察

目的:研究经鼻间隙正压通气(NIPPV)治疗早产儿原发性呼吸暂停反复发作的疗效。方法:胎龄小于34周的新入院早产儿原发性呼吸暂停反复发作患儿26例,随机分成两组,治疗组予NIPPV模式辅助通气。对照组先予氨茶碱针应用,若不能控制加用经鼻持续正压辅助通气(NCPAP)。记录分析两组患儿的呼吸暂停发生情况。结果:治疗组显效13例,显效率100%。对照组显效7例,有效2例,无效4例,总有效率69.2%。两组对比有统计学差异(P<0.05)。结论:NIPPV治疗早产儿原发性呼吸暂停反复发作效果明显好于氨茶碱加NCPAP疗法,可以避免因治疗失败而改用气管插管机械通气。

Observation on the curative effect of NIPPV in treatment of repeated attack of primary apnea among the premature infants

Objective:To research the curative effect of nasal intermittent positive pressure ventilation(NIPPV) in treatment of repeated attack of primary apnea among the premature infants.Methods:Twenty-six newly admitted premature infants with repeated attack of primary apnea whose gestational age were less than 34 weeks were divided into two groups randomly: the premature infants in treatment group were treated with assisted ventilation with NIPPV mode;while the premature infants in control group were treated with aminophylline at first,if the disease couldn’t be controlled,nasal continuous positive airway pressure(NCPAP) was applied.The occurrence of apnea in the two groups were recorded and analyzed.Results:In treatment group,13 infants had marked effect,the effective rate was 100%.In control group,7 infants had marked effect,2 infants had effect,4 infants had no effect,the total effective rate was 69.2%.There was significant difference between the two groups(P<0.05).Conclusion:The curative effect of NIPPV is better than that of aminophylline plus NCPAP in treatment of repeated attack of primary apnea among the premature infants,the application of NIPPV can avoid turning to trachea cannula due to treatment failure.
