宫腔内配子移植治疗黄素化未破裂卵泡综合征所致不孕症疗效观察

目的:观察宫腔内胚子移植(GIUT)治疗黄素化未破裂卵泡综合征(LUFS)所致不孕症的临床疗效。方法:对23例LUFS所致不孕症患者进行36个周期的GIUT,采用自然周期或小剂量氯米芬微刺激方案促排卵,阴道B超监测卵泡的生长情况,当有1个以上卵泡直径≥18或2个以上卵泡直径≥16时肌内注射人绒毛膜促性腺激素(HCG)5 000 IU,24～36 h内取卵实施宫腔内胚子移植。结果:36个周期GIUT中,临床妊娠7例,周期妊娠率19.44%,生化妊娠1例,36个周期中未发生宫外孕、双胎妊娠及卵巢过度刺激综合征(OHSS)。结论:GIUT治疗卵泡未破裂黄素化综合征所致不孕症有一定疗效,且程序简化,价格低,值得基层推广应用。

Observation on the curative effect of gamete intrauterine transfer in treatment of infertility induced by luteinizing unruptured follicle syndrome

Objective:To observe the curative effect of gamete intrauterine transfer(GIUT) in treatment of infertility induced by luteinizing unruptured follicle syndrome(LUFS).Methods:Twenty three patients with infertility induced by LUFS received GIUT(36 cycles),natural cycle or low-dose clomiphene microstimulation protocol was used for ovulation induction,transvaginal B ultrasonography was used to monitor the growth of follicles;when the follicular diameter of more than one follicle≥18 mm,or the follicular diameter of more than two follicles≥16 mm,the patients were treated with intramuscular injection of human chorionic gonadotropin(HCG) 5 000 IU;at 24-26 hours after HCG administration,GIUT was performed.Results:After GIUT for 36 cycles,seven patients got clinical pregnancy,the clinical pregnancy rate was 19.44%,one patient got biochemical pregnancy,no ectopic pregnancy,twin pregnancy,and ovarian hyperstimulation syndrome(OHSS) occurred during 36 cycles.Conclusion:GIUT has a certain curative effect in treatment of LUFS,which is simple and inexpensive,and the method is worthy to be popularized in primary hospitals.
