胰岛素泵持续皮下注射治疗妊娠期糖尿病对新生儿的影响

摘    要:目的:探讨胰岛素泵持续皮下注射胰岛素(CSII)治疗妊娠期糖尿病对新生儿的影响及其临床意义。方法:选择妊娠期糖尿病患者60例,其中30例应用胰岛素泵持续皮下注射治疗(CSII组),30例应用胰岛素多次皮下注射治疗(MSII组),比较两组患者的一般情况,治疗前、治疗后孕妇血糖值,新生儿出生后血糖及体重,计算并比较两组患者体重指数、治疗前后血糖、新生儿低血糖、巨大儿及大于胎龄儿的发生率。结果:两组在年龄、入组孕周、血糖及体重指数等方面差异均无统计学意义(P>0.05),而经治疗后CSII组平均血糖值低,新生儿低血糖及大于胎龄儿(LGA)发生率低,且分娩孕周晚,两组比较差异有统计学意义(P<0.05)。结论:胰岛素泵持续皮下注射比每日多次皮下注射胰岛素治疗妊娠期糖尿病降糖效果好,可延长孕周,新生儿低血糖及大于胎龄儿的发生率低,对于安全、有效地治疗妊娠期糖尿病,改善新生儿预后具有深远的临床意义。

Abstract:Objective:To explore the effects on newborns for the mothers with gestational diabetes mellitus treated by continuous subcutaneous insulin infusion(CSII)and its clinical significance.Methods:Sixty patients with gestational diabetes mellitus were choose.Thirty patients were treated by CSII,thirty patients were treated by MSII.General,blood sugar before and after treatment of patients and blood sugar and body weight of the newborns,neonatal hypoglycemia,the rate of macrosomia and LGA were recorded and compared in the two groups.Results:There was no significant difference between the two groups in age,gestational age into the group,blood glucose and body mass index,etc.And by continuous subcutaneous insulin pump therapy,the average blood sugar,the incidence of neonatal hypoglycemia and LGA were lower in CSII group,and gestational age was later,the difference between the two groups was statistically significant.Conclusion:Insulin pump is effective and safe for gestational diabetes mellitus,and has good hypoglycemic effect,can extend the gestational age,decrease the incidence of LGA and hypoglycemia of newborn,and has good prognosis to improve neonatal clinical outcomes.
