宫颈鳞癌中鳞状细胞癌抗原的相关作用探讨

目的:探讨鳞状细胞癌抗原(SCCAg)在宫颈鳞癌的临床疗效及复发监测中的意义。方法:选取2007年1月～2009年1月在广西医科大学肿瘤医院收治的宫颈鳞癌患者111例,用微粒子酶免疫分析技术测定治疗前后血清SCCAg滴度,结合临床资料对血清SCCAg水平与宫颈鳞癌患者临床疗效、复发和预后的关系进行多因素分析。结果:①治疗后宫颈鳞癌患者血清SCCAg阳性率为14.4%,较治疗前显著降低(P<0.05);Ⅰ-Ⅱ期患者治疗后血清SCCAg水平较Ⅲ-Ⅳ期患者显著降低(P<0.05);②复发患者治疗前血清SCCAg水平较未复发患者治疗前SCCAg水平显著升高(P<0.05)。③多因素分析显示,影响宫颈鳞癌复发预后的独立因素为血清SCCAg滴度、淋巴结转移和细胞分级。结论:血清SCCAg水平检测对宫颈鳞癌患者疗效判定、指导治疗、复发及预后的监测等方面均有重要的临床实用价值。

Clinical effect of Squamous cell carcinoma antigen in cervical cancer

Objective:To discuss the clinical effect of squamous cell carcinoma antigen(SCCAg) in cervical cancer and its significance in recurrence monitoring.Methods:The serum concentration of SCCAg was attained by microparticle Enzyme immunoassay(MEIA) in 111 patients with cervical cancer before and after clinical treatment from January,2007 to January,2009 at our hospital.Clinical data were used to investigate the correlation between the serum level of SCCAg and the clinical effect,the recurrence and prognosis through univariate and multivariate analysis.Results:①The positive rate of SCCAg in the post-treatment patients was 14.4%,which was significantly lower than that in the pre-treatment patients(60.0%)(P<0.05);after treatment,the serum level of SCCAg in stage Ⅰ-Ⅱ patients was significantly lower than that in stage Ⅲ-Ⅳ patients(P<0.05).②The serum level of SCCAg in the recurrent patients(12.05±13.31) increased significantly higher than that of the non-recurrent patients(5.94±5.63) before treatment(P<0.05).③Multivariate analysis revealed that the serum concentration of SCCAg,pelvic lymph node metastasis and cell grading were the factors affecting the recurrence and prognosis of cervical cancer.Conclusion:The serum level of SCCAg has the important clinical values as an adjunct method in monitoring cervical carcinoma status,assessing the efficacy,guiding treatment,detecting recurrence and predicting prognosis of cervical cancer.
