早发型重度子痫前期与终止妊娠的相关性探究

目的:探讨早发型重度子痫前期(EOSP)终止妊娠的时机和条件,最大限度地确保母婴安全。方法:将84例EOSP患者按发病孕周分为A(<28周)、B(28～32周)、C(32～34周)三组,按分娩方式分为阴道分娩和剖宫产两组,比较各组围生儿结局及孕产妇并发症发生率。结果:A、B、C三组孕产妇并发症发生率差异无统计学意义(P>0.05),但A组围生儿死亡率明显高于B组和C组(P<0.01);阴道分娩组母婴并发症发生率高于剖宫产组。结论:无严重并发症的EOSP可期待治疗至32周时终止妊娠,否则应立即行剖宫产以避免孕产妇终末脏器不可逆损害,降低婴幼儿死亡及病残率。

The study on the relationship between the early onset severe preeclampsia and termination of pregnancy

Objective:To investigate the timing and conditions of termination of pregnancy for the early onset severe preeclampsia(EOSP),in order to ensure the safe of motherhood.Methods:Eighty-four EOSP patients in our hospital were divided into three groups: group A(<28 weeks),group B(28-32 weeks),group C(32-34 weeks) according to the gestational age when incidence.At the same time,according to mode of delivery they were divided into vaginal delivery group and cesarean section group.The children perinatal outcome and maternal morbidity in each groups were analyzed.Results:The rates of maternal complication in group A,B,C were no significant difference(P>0.05),but perinatal mortality in children in group A was significantly higher than group B and C(P<0.01);The rate of maternal complication in vaginal delivery group was higher than in cesarean section group.Conclusion:EOSP with no serious complications can expectant to 32 weeks and then termination of pregnancy.If not cesarean section immediately and terminate the pregnancy,in order to avoid irreversible organ damage,and reduce the rate of infant and child mortality and morbidity.
