降纤酶对大鼠局灶性脑缺血的影响

摘    要:目的 :观察降纤酶 (defibrase)对大鼠局灶性脑缺血性损伤的治疗作用。方法 :雄性Wistar大鼠 ,采用大脑中动脉内线栓法诱导局灶性脑缺血 ,60只局灶性脑缺血大鼠 ,随机分成 1 0组 ,每组 6只 ,其中 5组大鼠分别在缺血后 0 .5 ,3 ,6,9,1 2h静脉注射降纤酶 ,8U·kg-1,另 5组大鼠分别在相同时间点静脉注射等量生理盐水对照 ,另设 1 0鼠作假手术组。各组大鼠均在手术后 2 4h进行神经功能评分 ,然后处死大鼠 ,标本用苏木精曙红染色 (HE)及氯化三苯基四氮唑(TTC)染色 ,观察梗死灶大小、病理改变。结果 :缺血后 0 .5 ,3h静脉给药治疗组神经功能评分比对照组明显改善 ,分别为 (0 .8±s 0 .5 )分 ,(1 .3± 0 .5 )分和 (3 .0± 0 .9)分 ,(3 .2± 1 .0 )分 ,脑梗死灶体积比对照组明显减小 ,分别为 (1 1 9±3 9)mm3 ,(1 41± 62 )mm3 和 (2 91± 5 6)mm3 ,(2 97± 3 9)mm3 ,差别有非常显著意义 (P <0 .0 1 ) ;缺血区病理改变也轻于对照组。缺血后 6,9,1 2h给药组神经功能评分、脑梗死灶体积与对照组比较均无明显差异 (P >0 .0 5 ) ,治疗组与对照组镜下均见点状出血 ,但未发现治疗组增加脑梗死后出血率。结论 :早期静脉应用降纤酶有效缩小脑梗死灶体积 ,且不增加出血率。

Abstract:AIM: To observe the therapeutic effect of defibrase after focal cerebral ischemia in male Wister rats. METHODS:Intraluminal suture method was used to develop reversible middle cerebral artery occlusion (MACO). Sixty focal cerebral ischemia rats were randomly divided into ten groups(one group six rats). Five groups were injected intravenously Hai wang defibrase 8 U·kg 1 at 0.5, 3,6,9,12 h after cerebral ischemia. The other five groups (control groups) were injected intravenously equivalent saline at the same time. Clinical neurological deficits scale were completed at 24 h after MCAO . Then all rats were killed. Cerebral infarction was examined microscopically in HE stained sections. Infarction volume was measured by using 2,3,5 triphenyltetrazolium chloride (TTC) staining. RESULTS:The rats were injected intravenously defibrase in 0.5, 3 h after MCAO. Their clinical neurological deficits scale was 0.8 ± s 0.5, 1.3±0.5, and 3.0±0.9, 3.2±1.0, respectively. The infarction volume was (119±39) mm 3, (141±62)mm 3, and (291±56)mm 3, (297±39)mm 3, respectively. Which were all significantly reduced in comparison with the control groups( P <0.01).Pathologic change was also significantly improved. There were no statistically significant differences at 6, 9,12 h after MCAO. Both treating groups and the control groups were found punctual hemorrhage under the microscop, but cerebral hemorrhage wasn't increased in defibrase treatment group. CONCLUSION: Defibrase can reduce the infarction volumn without the increase of hemorrhage at cerebral ischemia early stage.
