老年高血压患者动态脉压指数与颈动脉内膜-中层厚度的相关性探讨

摘    要:目的探讨动态脉压、脉压指数与颈动脉内膜-中层厚度的相关性。方法动态血压测量253例研究对象的动态脉压和脉压指数,按动态脉压≤40mmHg(59例)、4l-60mmHg(82例)、61-80mmHg(76例)及〉80mmHg(36例)分为4组;按动态脉雎指数≤0.40(62例)、0.41-0.50(86例)、0.5l-0.60(72例)及〉0.60(33例)分为4组。颈动脉内膜-中层厚度使用超声检测。比较不同动态脉压组及不同动态脉压指数组的颈动脉内膜-中层厚度。结果随着收缩压、舒张压、动态脉压和脉压指数的增加,颈动脉内膜-中层厚度随之增加,动态脉压、脉压指数与颈动脉内膜-中层厚度的相关系数r值大于收缩压和舒张压。结论动态脉压和脉压指数评价血管硬化的价值优于收缩压、舒张压,而动态脉压指数评价血管硬化比脉压更有优越性。

Abstract:Objective To investigate the correlation between dynamic pulse pressure index and carotid artery intima-media thickness. Methods Measuring the dynamic pulse pressure and pulse pressure index in 253 objects,then grouping them according to their dynamic pulse pressure:the first team : ≤40mmHg(59 objects), the second team:41 to 60mmHg(82 objects), the third team:61 to 80mmHg(76objects), the last team:〉80mmHg(36 objects) ;at the same time, grouping them according to their dynamic pulse pressure index:the first team: ≤0.40(62 objects), the second team:0.41 to 0.50 (86 objects), the third team:0.51 to 0.60 (72 objects), the last team:〉 0.60 (33 objects). The measurement of carotid artery intima-media thickness was determined by echocardiogr'aphy. Results Following the systolic pressure ,diastolic pressure,dynamic pulse pressure and pulse pressure index raising, the carotid artery intima-media thickness was increasing, the correlation coefficient r between dynamic pulse pressure,pulse pressure index and carotid artery intima-media thickness were higher than systolic pressure,diastolic pressure. Conclusions The dynamic pulse pressure and pulse pressure index surpass systolic pressure and diastolic pressure in estimating angiosclerosis,and pulse pressure index has an advantage to dynamic pulse pressure.
