前列腺癌患者自我形象及其影响因素的调查

目的调查前列腺癌患者对自我形象的满意情况，分析前列腺癌患者自我形象的影响因素。方法对2009年7月至2010年8月在上海市2所三级甲等医院泌尿外科门诊就诊的170例前列腺癌患者进行问卷调查，采用中文版《自我形象量表》测量前列腺癌患者的自我形象，同时填写个人一般资料和疾病相关资料。结果前列腺癌患者自我形象总分平均为（6．32±4．81）分，其中情感维度得分（3．36±2．34）分，行为维度得分（0．76±0．96）分，认知维度得分（2．19±1．88）分。多元回归分析显示，年龄是自我形象各个维度及总分的主要影响因素；而前列腺特异性抗原（PSA）控制、去势治疗和文化程度、临床分期分别是认知维度、情感维度及行为维度的影响因素。结论患者越年轻，疾病越晚期，PSA控制不好、接受过去势治疗、文化程度高，自我形象得分越高，患者对自我形象越不满意。应对前列腺癌患者提供积极有效的应对方案，以提高其生存质量。

Investigation on self- image and its influential factors of patients with prostate cancer

Objective To investigate body image in prostate cancer patients and explore the factors which influence the body image. Methods 170 men with prostate cancer were recruited from the urology outpatient departments at two hospitals in Shanghai. The participants completed the questionnaires, the body image scale (BIS) and the socio-demographic and disease-related instruments. Results The score of total BIS, emotion domain, active domain, cognitive domain were (6.32 ±4.81 ), (3.36 ± 2.34), (0. 76 ±0.96), (2. 19 ± 1. 88).The stepwise multiple regression analysis showed that the age, control of PSA, the androgen deprivation therapy (ADT), education level, and disease stage were significant predictors of BIS. Conclusions The participants with younger age, received ADT, bad control of PSA, receive less education, more advanced disease stage had less satisfactory in self-body image. So nurses should respond to positive and effective response options to prostate cancer patients,to improve their quality of life.
