复杂髋臼骨折的手术治疗

目的探讨复杂髋臼骨折的治疗策略。方法总结本院2002～2007年经X线片、三维CT重建诊断的34例有移位复杂髋臼骨折治疗的经验。结果34例患者获得随访,平均随访时间为2.7（1.2～5）年。根据Matta解剖学评分,解剖复位20例,满意复位12例,不满意复位2例。根据Harris功能评定标准,优27例,良4例,可2例,差1例。结论完善的影像学资料,正确的术前诊断,适当的手术时期和手术方法,解剖复位,早期功能锻炼,降低并发症的发生可提高复杂髋臼骨折的治疗效果。

The Surgical Treatment of Complex Acetabular Fractures

Objective To discuss the treatment strategies of complex aceiabular fractures.Methods The experience in treatment of 34 cases of complex acetabular fractures treated operatively from 2002 to 2007 was summarized.They were diagnosed by X-ray,CT and 3-D CT.Results 34 cases were followed up from 1.2 to 5 years with an average time of 2.7 years.They were evaluated according to Mafia anatomic criteria,20 cases with anatomic reduction, 12 cases with satisfied reduction,2 cases with unsatisfied reduction, and evaluated by Harris criteria for joint functions,27 cases with excellent,4 cases with fine,2 cases with fair, 1 cases with poor. Conclusion Good image data,correct preoperative diagnosis,appropriate operation time and approach,anatomical reduction,early functional rehabilitation,decrease of complication are essential for better treatment result s of complex acetabular fractures.
