电解脱铂金弹簧圈栓塞治疗颅内动脉瘤

目的：总结电解脱铂金弹簧圈（EDPC）治疗颅内动脉瘤的临床体会，对其指征、操作要点及主要并发症的防治进行初步探讨。方法：2004年3月至2005年1月用EDPC栓塞治疗17例患者19枚颅内动脉瘤，其中14例表现为自发性蛛网膜下腔出血（SAH）。经血管造影证实后，在微导丝导引下微导管超选动脉瘤腔填塞弹簧圈。结果：2例死亡，1例于术后4d再破裂后完全填塞，其余14例患者16枚动脉瘤中12例完全填塞，90％以上填塞者2例，60％～70％填塞者2例，载瘤动脉通畅。结论：电解脱弹簧圈栓塞术是治疗颅内动脉瘤的一种有效方法；通过材料和技术的改进．可以进一步提高其疗效．降低并发症。

Treatment of cerebral aneurysms with electro-detachable platinum coils

Objective: To summarize our clinical experience of treatment of cerebral aneurysms with(Electro-)detachable platinum coils(EDPC). Methods: From March 2004 to January 2005,17 patients with 19 aneurysms were embolized with EDPC,including 14 ruptured aneurysms.Microcatheters were introduced into the aneurysm sac,EDPC were used to embolize aneurysms. Results: Two patients died and one patient was treated with coils again because of recanalization of the aneurysm.Of the other aneurysms,total occlusion was achieved in 12 aneurysms,over 90% in 2 aneurysms,and 60%-70% in 2 aneurysms.The patients recovered well with patency of the parent arteries. Conclusion: Endovascular therapy with EDPC placement is a valid approach for the treatment of cerebral aneurysms.With the development of interventional therapy,it may play more important roles in the treatment of cerebral aneurysm.
