浙江舟山普陀渔区20年脑卒中发病变化的前瞻性研究--脑卒中保护因子初探

目的研究舟山普陀渔区脑卒中 2 0年的动态变化及对低发原因的初探。方法按中国医学科学院心血管病研究所编写的《心血管病流行病学调查研究手册》的统一要求和表格 ,从 1982年起至 2 0 0 1年止 ,对近 10万纯渔区人群进行脑卒中发病和死亡的人群监测。结果在脑卒中诸多已知危险因素中 ,高血压乃属最主要、独立的危险因素。舟山普陀渔区人群脑卒中总发病率较国内均值明显为低 (2 0 .73/ 10万 )。结合以往本地区的有关报道 ,其保护因素可能包括 :渔区人群海产鱼蛋白摄入量高、多不饱和脂肪酸摄入量高 ;宏量元素钙、镁增高 ,微量元素锌增高及镧降低 ;载蛋白 B水平低等。结论舟山普陀渔区为脑卒中低发区 ;在研究脑卒中发病危险因素的同时 ,也必须深入研究某些保护因素的作用 ,以利脑卒中的防治

A Prospective Survey of the Change of Stroke Incidences over 20 Years in Putuo Fishing Village of Zhoushan,Zhejiang Provinc—— A Primary Exploration on the Protective Factors for Stroke

Objective To study the change of stroke incidences over 20years in Putuo fishing village of Zhoushan,Zhejiang province and to make a primary research on the causation of its low incidence.Method According to the regulations of Handbook of Survey Methods of Cardiovascular Epidemiology made by Cardiovascular Institute of Chinese Medical Academy,the incidence and mortality rates were prospectively monitored in about 100 000 fishing population from 1982 to 2001.Results Hypertension was the most important and independent risk factors for stroke.In comparison with the average incidence of the nation,the total incidence of stroke in this district was lower(20.73/100 000).The protective factors for stroke in this village included:high intakes of sea fish,protein,polyunsaturated fatty acids,macroelement calcium and magnesium,and microelement Zinc;low intake of Lanthanum,and low apolipoprotein B level. Conclusion Zhoushan Putuo is a fishing district with low incidence of stroke.In order to prevent and cure stroke ,a further exploration of its protective factors should be made as well as the study of its risk factors.
