急性脑梗死后认知障碍与血尿酸相关性的多因素分析

摘    要:目的探讨急性脑梗死后认知障碍与血尿酸相关性的多因素分析。方法对2012-12—2014-12我院收治的113例急性脑梗死患者的临床资料进行回顾性分析,根据MoCA测试将患者分为认知障碍者和无认知障碍者,对患者急性脑梗死后认知障碍与血尿酸相关性进行多因素分析。结果 113例急性脑梗死患者有认知障碍76例,无认知障碍37例,患者性别、接受高等教育、糖尿病、血同型半胱氨酸水平、血尿酸、关键部位梗死是急性脑梗死后认知障碍的相关因素(P0.05);对急性脑梗死后认知障碍有关的因素进行多因素Logistic回归分析得出患者性别、接受高等教育、糖尿病、血同型半胱氨酸水平、血尿酸、关键部位梗死是急性脑梗死后认知障碍的重要影响因素(P0.05);急性脑梗死后认知障碍与血尿酸相关因素进行多因素Logistic回归分析,男性、中青年、小动脉闭塞型高尿酸血症患者急性脑梗死后认知障碍风险更大,差异具有统计学意义(P0.05)。结论高尿酸血症是急性脑梗死后认知障碍的独立影响因素,且男性、中青年、小动脉闭塞型急性脑梗死患者认知障碍风险更大。

Multi-factor correlation analysis of cognitive impairment after acute cerebral infarction
Abstract:Objective To investigate multi-factor correlation analysis of cognitive impairment after acute cerebral infarc‐tion.Methods The clinical data of 113 cases with acute cerebral infarction from December 2012 to December 2014 in our hos‐pital were retrospectively analyzed ,who were divided into cognitive impairment group and non-impairment group according to MoCA test.Then we performed multivariate analysis between cognitive impairment and the potential factors related with serum uric acid.Results Of the selected 113 cases ,there were 76 cases with impairment group and 37 cases without impairment.Our analysis showed that gender ,higher education ,diabetes ,blood homocysteine ,serum uric acid and key location of infarction were the important influence factors for cognitive impairment(P<0.05).Logistic regression analysis presented that in hyperurice‐mia group ,male ,young and mid-aged and small artery occlusion were regarded as higher risk factors that made cognitive disor‐der easier in patients with post-acute cerebral infarction(P<0.05).Conclusion:Hyperuricemia may be the independent risk fac‐tor for cognitive impairment after cerebral infarction.And male ,young and mid-aged ,small vessel occlusion can become the higher risk factors .
