罗格列酮对急性坏死性胰腺炎及合并肺损伤的防治作用研究

摘    要:目的研究罗格列酮(ROSI)对急性坏死性胰腺炎(ANP)及合并肺损伤大鼠的保护作用。方法 40只SD大鼠随机分为ANP组、预防组(ROSI预防组)、治疗组(ROSI治疗组)和对照组各10只。ANP组给予腹腔注射精氨酸造模,预防组于造模前注射ROSI,治疗组于造模后注射ROSI,对照组在尾静脉注射同等剂量生理盐水。各组于造模后24h观察大鼠血清淀粉酶(AMY)、肿瘤坏死因子(TNF-α)、白介素-1β(IL-1β)、血清抵抗素、血清高敏C-反应蛋白(Hs-CRP)水平变化,同时检测胰腺和肺的组织病理变化和肺组织髓过氧化物酶(MPO)和肺湿干比(W/D)的变化。结果 ANP组AMY、TNF-α、IL-1β、血清抵抗素、Hs-CRP和MPO、W/D均较对照组明显升高(P〈0.01),ROSI预防组和治疗组血清AMY、TNF-α、IL-1β、血清抵抗素、Hs-CRP及肺组织MPO、W/D水平较ANP组明显下降,且胰腺和肺组织病理损伤较ANP组也明显减轻(均P〈0.01)。结论 ROSI能明显降低ANP的炎症,对ANP及合并的肺损伤具有保护和治疗作用。

Abstract:Objective To study the protective effect of rosiglitazone(ROSI) on acute necrotic pancreatitis(ANP) and pancreatitis associated pulmonary injury.Methods 40 Sprague Dawley(SD) rats were randomly divided into ANP group,prophylactic group,therapeutic group and control group,10 cases of each.ANP group was given intraperitoneal injection of L-arginine to induce ANP model;The prophylactic group was given ROSI before inducing ANP model;The therapeutic group was given ROSI after induction of ANP model;The control group was injected into the same volume of 0.9% saline solution.Observed the amylase(AMY),TNF-α,IL-1β,resistin,Hs-CRP in serum,and myeloperoxidase(MPO) activity,wet/dry ratio(W/D) were evaluated.Results The levels of serum AMY,TNF-α,IL-1β,resistin,Hs-CRP,lung MPO,W/D ratio in ANP group were significantly higher than those of the control group(P 0.01) ;Compared with those of ANP group,the levels of serum AMY,TNF-α,IL-1β,resistin,Hs-CRP,lung MPO,W/D ratio of prophylactic and therapeutic group were significantly lower(P 0.01),while pancreatic and pulmonary pathological injury significantly reduced(P 0.01).Conclusion ROSI can significantly reduce the inflammation of ANP,and also have protective and remedial function on ANP and pancreatitis associated pulmonary injury.
