丙泊酚联合舒芬太尼在小儿先天性心脏病介入治疗中的应用

目的评价小儿先天性心脏病介入封堵术中联合应用丙泊酚和舒芬太尼的临床效果。方法先天性心脏病患儿80例,术前用阿托品0.02mg/kg,入导管室后建立静脉通道,丙泊酚1.5mg/kg和舒芬太尼0.1μg/kg诱导麻醉,继以丙泊酚50μg.kg-1.min-1的速度持续泵入。连续监测并记录无创血压、心电图、脉搏、血氧饱和度（SpO2）、心率（HR）、呼吸频率（RR）,观察造影检查封堵的即刻效果。结果整个麻醉手术过程中,SpO2无明显变化（P〉0.05）。手术期间置入导管、进行心内操作时RR和MAP较麻醉前显著升高,差异有统计学意义（P〈0.05）;停药后术后苏醒时间平均为8min,苏醒时间与麻醉手术时间无相关性。苏醒时HR、MAP和RR较麻醉前无明显变化（P〉0.05）。且无明显并发症发生。结论丙泊酚复合舒芬太尼麻醉诱导可用于小儿先天性心脏病的介入治疗,其诱导迅速,作用平稳,术后苏醒快,是一种良好的麻醉方法 。

Clinical observe the effect of propofol combined with sufentanil in intervening therapy for congenital heart diseases in children

Objective To evaluate the efficacy of propofol combined with sufentanil in intervening therapy for congenital heart diseases in children.Methods 80 cases patients of congenital heart diseases were given atropine 0.02mg/kg before operation,and then induced with propofol 1.5mg/kg and sufentanil 0.1μg/kg.The anesthesia was maintained by pumping propofol at a rate of 50μg·kg-1·min-1.The indexes of blood pressure,ECG,pulse,SpO2,HR and RR were monitored through the course of the closure procedure.Results The whole of the operation,SpO2 had no apparent change（P0.05）.During the operation in tube and on operation,the RR and MAP were higher than before operation,there had significant differences（P0.05）;After surgery to the average time was 8min,wake-up time and the operation time had no relevance.At wake-up,HR,MAP and RR compared with preoperation,had no marked change（P0.05）.And without complications occured.Conclusion Propofol combined with sufentanil is a safe anesthesia method in intervening therapy for congenital heart diseases in children.
