抑肽酶对骨肉瘤手术患者围术期凝血功能的影响

目的探讨抑肽酶对骨肉瘤手术患者围术期凝血功能的影响。方法骨肉瘤手术患者20例随机分为抑肽酶组和对照组,各10例。抑肽酶组抑肽酶首量1×106kU,维持量0.25×106kU/h持续静脉泵注射至术毕,对照组泵注0.9%氯化钠溶液。手术前和手术开始后1h采集颈内静脉血检测凝血功能,包括凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）、凝血酶时间（TT）、纤维蛋白原（FIB）、血栓弹力扫描仪（TEG）的反应时间（R）等。结果手术开始后1h抑肽酶组患者的PT、APTT、TT、R均较对照组短,差异有统计学意义（P〈0.05）。结论骨肉瘤手术中应用抑肽酶能有效改善围术期凝血功能,减少术中出血量。

Effect of aprotinin on blood coagulation and dibrinolysis sestem during perioperative period of osteosarcoma patients

Objective To investigate the effect aprotinin on blood coagulation and dibrinolysis sestem during perioperative period of osteosarcoma patients.Methods 20 cases of perioperative period osteosarcoma patients were randomly divided into aprotinin group and control group,each of 10 cases.The first volume of aprotinin group was 1×106kU,continuous infusion was 0.25×106kU/h,continuous intravenous aprotinin,to completion of surgery.The control group infusioned 0.9% sodium chloride solution.Prior to surgery and collected 1h after the beginning of detection of carotid blood coagulation,including prothrombin time（PT）,activated partial thromboplastin time（APTT）,thrombin time（TT）,fibrinogen（FIB）,reaction time（R）of thrombosis stretch scanner（TEG）,etc.Results After the beginning of surgery 1h,the PT、APTT、TT、R of aprotinin group were shorter than that in control group,the difference was significantly（P0.05）.Conclusion Aprotinin used in perioperative period of osteosarcoma patients,can improving coagulation and dibrinolysis.
