胃肠减压在直肠低位切除术后快速流程中的前瞻性对照研究

目的研究直肠低位切除术术后早期拔除胃管与肠道功能早期恢复的关系。方法前瞻性分析2009年6月至2010年5月期间四川大学华西医院胃肠外科直肠癌患者141例,采取随机数字表法分为快速流程（FT）组（57例）和传统组（84例）。比较两组之间康复指标的差异。结果两组康复指标的比较发现,在胃管拔除时间、饮水试餐时间、半流质饮食时间、普食时间、血浆管拔除时间、尿管拔除时间、术后住院天数等方面,差异均有统计学意义,且快速流程组均比传统治疗组早;同时pearson相关分析发现,饮水试餐时间（P=0.000）与半流质饮食时间（P=0.000）均与胃管拔除时间有很好的相关性。结论术后早期拔除胃管可以提高患者预后。

The Prospective Randomized Control Trial of Gastrointestinal Decompression for Low Rectal Cancer Resection in Fast Track

Objective To study the relationship between the early remove of stomach-tube and the intestinal tract＇s early recovery after the low rectal resection.Methods 152 patients diagnosed definitely as rectal cancer at West China Hospital of Sichuan University from 2009 to December 2009 were randomly assigned into two groups randomly.One group was Fast Truck group,another group was Conventional group.Results The differences between the two group of the gastric tube removal time,drinking water test meal time,semi-liquid diet time,normal diet time,plasma tube extraction time,catheter removal practice,postoperative hospital stay and so on were statistically significant.And those in the Fast Truck group were earlier than those in the Conventional treatment group.Among the water test meal time（P=0.000）,semi-liquid diet time（P=0.000）and early remove of stomach-tube has a good correlation time by the pearson correlation analysis.Conclusion Early remove of stomach-tube postoperatively is related to a better prognosis.
