急性心肌梗死阿托伐他汀治疗对高敏C-反应蛋白及TC:HDLC的影响

摘    要:目的:研究急性心肌梗死(AMI)患者早期给予不同剂量阿托伐他汀治疗对高敏C反应蛋白及TC:HDLC(总胆固醇与高密度脂蛋白胆固醇的比值)的影响。方法:103例AMI接受阿托伐他汀调脂治疗,根据剂量不同随机分2组,常规剂量组(n=51,10mg/d)和较高剂量组(n=52,20mg/d)。治疗前及治疗14d后测量血清高敏C反应蛋白(hs-CRP)、血脂及肝功能等指标。结果:治疗14d后,两组hs-CRP与治疗前相比均有显著下降(P<0.05),较高剂量组hs-CRP下降更明显(P<0.01);两组LDL-c、TG和TC:HDLC均有下降,但未达到统计学差异(P>0.05)。结论:急性心肌梗死患者早期给予阿托伐他汀治疗,短期就有抗炎作用,且抗炎作用独立于调脂作用,较高剂量的阿托伐他汀治疗可能获益更大。

Abstract:Objective: To investigate the influence of atorvastatin on hs-CRP and TC:HDLC in patients with acute myocardial infarction.Method: 103 patients with AMI were randomly divided into standard-dosage atorvastatin group(n=51) and high-dosage atorvastatin group(n=52),once daily atorvastatin 10mg and 20mg were given respectively.The serum high sensitivity C-reactive protein,lipid profile and liver function were observed at baseline and 14 days after atorvastatin treatment.Result: Compared with the baseline,the serum hs-CRP were significantly reduced in both groups(P<0.05 in standard-dosage group and P<0.01 in high-dosage group),the serum level of LDL-c,TG and TC:HDLC were reduced in both group,but it was not significantly(P>0.05).Conclusion: In patients with acute myocardial infarction,anti-inflammatory effects were observed in the early stage of statin treatment,independent of its lipid-lowering effects.The high-dosage atorvastatin was more effectively than the standard-dosage atorvastatin in patients with acute myocardial infarction.
