腰椎间盘突出合并侧隐窝狭窄症87例的诊治

目的 探讨腰椎间盘突出合并侧隐窝狭窄症诊断和治疗中的有关问题。方法 全部病例均行腰椎间盘髓核除和侧隐窝扩大手术。根据不同的病理特点。分别采用开窗式、半推板、全椎板切除等方法。结果 术后经1年至5年个8月的随访，优55例，良26例，进步4例，差2例，优良率93.1%。结论 在诊断本病时影偈学检查必须与临床检查相结合，手术除椎间盘后需扩大狭窄的侧隐窝，在彻底减压的同时注意保护脊柱的稳定性。

Lumbar disc herniation with lateral recess stenosis A report of 87 cases

Objective To discuss the problems encountered in diagnosing and treating lumbar disc herniation with lateral recess stenosis.Methods All patients were treated by lumbar disc extraction and enlargement of the lateral recess.According to the various pathologic condition,surgical intervention were accomplished by one of the following approches:laminal fenestration,hemilaminectomy or total laminectomy.Results All patients were followed up from one year to five years and eight months.The results were excellent in 55 cases,good in 26 cases,fair in 4 cases and poor in 2 cases.The excellent and good cases was 93.1%.Conclusion The findings of image examination must be considered with the findings of clinical examination.The lateral recess should be enlarged during extraction of the herniated lumbar intervertebral disc.It is important to protect the lumbar spinal stability when decompressed thoroughly.
