摘    要:目的:探讨质子泵抑制剂(PPI)与他汀类药物对氯吡格雷作用的影响。方法:查阅国内外文献,综述氯吡格雷与PPI或他汀类药物之间相互作用的情况。结果:PPI和部分他汀类药物对氯吡格雷的影响体现在代谢过程中对CYP2C19和CYP3A4的竞争和抑制,以及CYP450的基因多态性上。同时服用此3类药物的患者抗血小板作用可能无法达到预期效果。结论:就氯吡格雷、PPI和他汀类药物三者之间相互作用而言,建议错时服用或选用对氯吡格雷影响较小的药物。

Abstract:Objective: To investigate the impact of proton pump inhibitor (PPI) and statin drugs on the role of clopidogrel. Methods:The interaction of clopidogrel and PPI or statin drugs is overviewed by reading the literatures at home and abroad. Results:The impact of PPI and some statins drugs on the role of clopidogrel is relfected by the competition and suppression of CYP2C19 and CYP3A4 in the metabolic process and the genetic polymorphisms of CYP450 as well. Antiplatelet effect may not achieve the desired results when patients take three kinds of those drugs at the same time. Conclusion:In view of the interaction effect among clopidogrel, PPI and statins drugs, it is recommend that staggering the time for taking those medicine or choosing the drugs with smaller inlfuence on clopidogrel.
