男性肺癌患者外周血性激素水平及其与肺癌组织中芳香化酶表达的相关性研究

摘    要:背景与目的:雄激素转化为雌激素是由芳香化酶催化的,目前已知芳香化酶是该反应的最后一步限速酶。已有研究提示肺癌患者有性激素的失衡和紊乱,但其原因尚未清楚。因此,本文旨在探讨男性肺癌患者外周血性激素水平及其与肺癌组织中芳香化酶表达的相关性。方法:采用磁性微粒分离的免疫酶联分析测定方法检测25例男性肺癌患者和30例正常男性外周血血清中雌二醇(estradiol,E2)和睾酮(testosterone,T)水平,同时应用免疫组织化学方法检测25例肺癌组织及其癌旁组织和11例肺良性病变组织中芳香化酶蛋白表达。结果:25例男性肺癌患者外周血血清中E2水平较正常男性明显增高(P<0.01),T明显降低(P<0.01);肺癌组织中芳香化酶的蛋折表达阳性率为52%,而癌旁组织及肺良性病变组织均为阴性;外周血血清中E2水平与肺癌组织中芳香化酶的蛋白表达水平呈正相关(γ=0.9308,P<0.001)。结论:肺癌组织中表达的芳香化酶催化雄激素转化为雌激素并释放入血,可能是导致男性肺癌患者外周血中雌激素升高、雄激素降低的主要原因。

Abstract:Background &Objective:The conversion of androgen to estrog en is catalyzed by aromatase.Eviden ce to date indicates aromatase is the last rate-limited enzyme in this reactio n.Previous studies have suggested t hat there is some imbalance and abnormality of sexual hormone of patients with lung cancer,but the reason is not clear yet.This study was designed to evaluate the relationsh ip between level of sexual hormone of external blood and aromatase expression in cancer tissues of male patients with lung ca ncer.Method:The levels of estradiol(E 2 )and testosterone(T)in plasma of external blood of25patients with lung cancer and 30healthy men were measured by Enzyme Immunoassay Magnetic Soli d Phase(IEMA).The protein expression of aromatase in 25cancer tissues,25 juxtacancerous tissues,and 11tissues with benign lung disease were detected by Immunohistochemistry.Results:The level of plasma E2 in external blood was significantly increased(P<0.01)while T was significantly decreased(P<0.01)in male patients with lung cancer.The e xpression of aromatase in about 52%l ung cancer tissue samples was positi ve,but no aromatase expression in the juxtacancerous tissues and tissues with ben ign lung disease were found.The level of E 2 had posi-tive correlation to the expression o f aromatase in male lung cancer patie nts(r=0.9308,P<0.001).Conclusion:It is sug-gested that releasing of aromatase o f lung cancer tissue,which catalyze s androgen to change into estrogen,i nto external blood might be the major reason of the incre ase of E2 and the decrease of T in external blood.
