术前彩超在诊断分化型甲状腺癌早期颈转移中的作用

目的研究术前彩超在诊断分化型甲状腺癌早期颈转移中的作用。方法将潼南县人民医院80例患者随机分为研究组和对照组两组,其中研究组患者为术前颈部触诊为阴性,由彩超检查诊断为甲状腺癌颈转移的32（35侧）例患者,对照组为术前颈部触诊为阳性,诊断为甲状腺癌颈转移的48（50侧）例患者,两组患者都受过改良型颈清扫术,同时比较两组患者术前和术后的病理情况。结果由彩超诊断为颈转移的32例患者中,术后病理有25侧为颈转移,彩超检查的灵敏度为89%。并且彩超发现有66%的转移淋巴结为多分区转移,其中中颈为最高。病理发现有76%的转移淋巴结为多分区转移,通过研究发现病理和彩超的检查结果基本相同。结论术前彩超可以提高诊断分化型甲状腺癌早期颈转移的诊断率,并且对转移淋巴结进行准确定位。

Effect of Pre-operation Color Doppler Ultrasound in Diagnosis of Early Stage Cervical Metastasis of Differentiated Thyroid Carcinoma

Objective To evaluate the effect of pre-operation color Doppler ultrasound in diagnosis of early stage cervical metastasis of differentiated thyroid carcinoma. Methods Eighty patients admitted in Tongnan County People＇s Hospital were randomly divided into control group and research group. The research group included 32（ 35 sides） patients of negative cervical palpation and diagnosed as thyroid cancer metastasis by color Doppler ultrasound; the control group included 48（ 50 sides） patients of positive cervical palpation and diagnosed as thyroid cancer metastasis by color Doppler ultrasound. The two groups both received modified neck dissection and the pathologic conditions were compared pre and after the operation. Results Of the 32 patients with cervical metastasis diagnosed by color Doppler ultrasound,25 cases showed pathological cervical metastasis,with a sensitivity of 89%; 66% of metastatic lymph nodes were of multi-partition transfer,with the highest portion in the middle of the neck. By pathological examination,76% of the metastatic lymph nodes were found as multi-partition metastasis. The results with color Doppler ultrasound and pathological examination were basically consistent. Conclusion Pre-operation color Doppler ultrasound can improve the diagnostic rate of early stage cervical metastasis of differentiated thyroid carcinoma and could pinpoint the transferred lymph gland.
