摘    要:目的探讨多重聚合酶链反应(MPCR)快速诊断感染性眼内炎的临床应用价值。方法感染性眼内炎患者38例(42眼),依据感染性眼内炎常见细菌、真菌共有引物片段建立的MPCR方法检测,结果与常规细菌和真菌培养方法相比较。结果 42份样本中,MPCR检出细菌阳性28份、真菌阳性8份、混合感染2份,检出率90.5%(38/42);常规培养方法检出26份细菌性阳性、真菌性9份、混合感染2份,检出率88.1%(37/42);两种方法的阳性菌检出率差异无统计学意义(P>0.05)。结论 MPCR与常规方法均有较高的特异性和灵敏度;但MPCR检测花费时间短,有利于感染性眼内炎的快速诊断。

Abstract:Objective To investigate the application value of rapid diagnosis of infectious endophthalmitis using multiplex polymerase chain reaction(MPCR).Methods MPCR tecnique was used to differentiate the DNA segments of common bacteria and fungi in 38 patients(42 eyes) with infectious endophthalmitis,the results were compared to those reported by conventional culture.Results Of 42 samples,MPCR revealed bacteria and(or) fungi in 38 samples(bacteria in 28,fungi in 8,and both bacteria and fungi in 2) with a total positive rate of 90.5%.While conventional bacterial and fungal culture revealed bacteria and(or) fungi in 37 samples(bacteria in 26,fungi in 9,and both bacteria and fungi in 2) with a total positive rate of 88.1%(P>0.05).Conclusion Both of MPCR detection and conventional bacterial and fungal culture have similar advantages of higher sensibility and specificity in detecting common bacteria and fungi in the patients with infectious endophthalmitis.Compared to the later,MPCR detection is quicker and helpful in rapid diagnosis.
