超声实时弹性成像对乳腺良恶性病灶的鉴别诊断价值

目的探讨超声实时弹性成像对乳腺良恶性病灶的鉴别诊断价值。方法对行乳腺肿物切除的125例患者的139个病灶行超声实时弹性成像检查,同时测量乳腺病灶与同一深度乳腺组织的应变率比值。结果乳腺良恶性病灶在实时弹性成像评分及应变率比值方面比较,差异均有统计学意义(P<0.01)。弹性成像评分鉴别诊断乳腺病灶良恶性的灵敏度、特异度、阳性预测值、阴性预测值分别为84.21%、95.83%、76.19%、97.46%。应变率比值鉴别诊断乳腺良恶性病灶的灵敏度、特异度、阳性预测值、阴性预测值分别为84.21%、86.67%、50.00%、97.20%。实时弹性成像评分与应变率比值在鉴别诊断乳腺良恶性病灶方面具有相关性。结论超声实时弹性成像在乳腺良恶性病灶鉴别诊断中具有较高的临床应用价值。

The value of ultrasonic real-time elastography in differential diagnosis of breast lesions

Objective To explore the value of ultrasonic real-time elastography(URTE) in the differential diagnosis of benign and malignant breast lesions.Methods A total of 125 patients(139 lesions) was examined with URTE.The strain ratio of breast lesion to the breast tissues in the same depth was measured as the reference.The elastographic images were analyzed with the five-point elastic score and strain ratio simultaneously.Results There were significant differences between benign and malignant breast lesions in the elastography score and strain ratio(P<0.01).The sensitivity,specificity,positive predictive value and negative predictive value of elastography score for diagnosing malignant breast lesions were 84.21%,95.83%,76.19% and 97.46%,respectively,which of strain ratio for diagnosing malignant breast lesions were 84.21%,86.67%,50.00% and 97.20%,respectively.Elastography score and strain ratio were significantly correlated in the differential diagnosis of benign and malignant breast lesions.Conclusion URTE is highly valuable in the differential diagnosis of benign and malignant breast lesions.
