心力衰竭患者血红蛋白水平变化与心功能关系的研究

目的:观察充血性心力衰竭(CHF)患者血红蛋白水平变化及其与心功能的关系.方法:对入选的121例CHF患者测定了血红蛋白,超声心动图测量左室射血分数(LVEF),评价心功能;按血红蛋白水平分为CHF贫血组和非贫血组,同时选择27例正常人作为对照组.结果:CHF患者Hb水平和LVEF低于对照组(P＜0.01);随着心功能恶化及LVEF的降低,Hb水平逐级降低,贫血组CHF患者LVEF明显低于非贫血组CHF患者和健康对照组(P＜0.01);随着贫血程度的加重,CHF患者LVEF进一步降低(P＜0.01);相关分析表明,Hb与LVEF呈正相关.结论:CHF患者Hb水平的降低参与了心功能不全发生发展的病理生理过程,而CHF患者贫血的出现使心功能不全程度更加恶化.

Relationship Between Hemoglobin Concentration and Cardiac Function in Patients with Congestive Heart Failure

Objective: To study the change of a hemoglobin (Hb) concentration and its relationships with cardiac function in patients with congestive heart failure (CHF). Methods:121 CHF patients were designed in the anemia group and non-anemia group; 27 healthy subjects as serve the control group. The level of Hb was measured and the left ventricular ejection fraction (LVEF) was recorded by echocardiography.Results:37.2 % of 121 CHF patients had anemia. The level of Hb and LVEF were lower (P<0.01) in the CHF group than that in the control group. The level of LVEF in the anemia group was lower (P<0.01) than that in the non- anemia and the control group following the decrease of Hb. The results showed that there was a positive correlation between Hb concentration and the LVEF (P<0.01).Conclusions:Decrease of Hb could associate with cardiac dysfunction on patho-physiology and the anemia episode accelerated the development of cardiac dysfunction in patients with CHF.
