99mTc-MIBI首次通过法心功能测定和心肌显像对心肌梗死后左室重构的研究

摘    要:目的:评估急性心肌梗死后左室重构的状况,判断其预后价值。方法:对46例急性心肌梗死(AMI)患者(研究组)与10例健康者(对照组),采用99mTc-MIBI的首次通过法表模式采集心功能测定与心肌灌注显像同步进行的方法,研究其心肌血流灌注,左心室大小、形态及功能的改变。结果:心肌灌注显像显示,46例AMI患者梗死心肌节段均呈现面积大小不等的放射性缺损或减低,其中26例显示单支冠脉病变,20例显示多支冠脉病变;心功能测定显示,左室射血分数、心室扩大及室壁运动的改变程度与左室重构程度密切相关(P<0.05~<0.01)。结论:采用99mTc-MIBI的首次通过法表模式采集心功能测定与心肌灌注显像同步进行的方法,能较全面地反映心肌梗死后的左室重构情况,对评估疗效及判断预后有较高的临床价值。

Abstract:Objective: To quantitatively evaluate LVRM (left ventricle remodeling) following acute myocardial infarction (AMI) detected by myocardial perfusion imaging combining with cardiac function measuring with list mode acquiring & first pass method of 99mTc-MIBI (methoxyisobutyl isonitrilet). Methods: A total of 46 patients with AMI and 10 healthy people were studied the change of myocardial perfusion. shape and structure in the left ventricle by myocardial perfusion imaging combining with cardiac function measuring with list mode acquiring & first pass method of 99mTc-MIBI. Results: Myocardial perfusion imaging showed that different size radioactive lack or diminution occurred in 46 patients, 26 of them were demonstrated single coronary artery disease. 20 of them were demonstrated several coronary artery disease. Cardiac function measured by 99mTc-MIBI showed the changing degree of LVEF, ventricle enlarge and the wall motion well correlated with the degree of LVRM (P<0. 05 - <0. 01). Conclusion: Myocardial perfusion imaging combining with cardiac function measuring with list mode acquiring &. first pass method using 99mTc-MIBI can relative roundly evaluate LVRM and possesses higher clinical value on evaluating therapeutic efficacy and prognosis.
