大剂量醛氢叶酸加5-FU持续48小时滴注联合顺铂方案治疗晚期鼻咽癌的临床观察

目的评价大剂量醛氢叶酸加5氟尿嘧啶(5-FU)持续48h滴注联合顺铂(DDP)方案治疗晚期鼻咽癌的客观疗效及毒副反应,探索晚期鼻咽癌较合理治疗方案.方法对入选的30例晚期鼻咽癌病人采用大剂量醛氢叶酸+5FU持续48h滴注联合顺铂方案进行治疗,平均2.7个疗程.结果完全缓解(CR)4例,部分缓解(PR)18例,稳定(NC)7例,进展(PD)1例,有效率为73.33%.主要毒副反应为恶心呕吐、口腔粘膜炎、骨髓抑制及脱发.上述毒副作用除恶心呕吐较明显外,其余绝大多数均为工～Ⅱ度反应,且发生率不高,经常规对症治疗后见好转.结论大剂量醛氢叶酸+5FU持续48h滴注联合顺铂方案治疗晚期鼻咽癌疗效好,毒副反应轻,患者普遍可以耐受.

High-dose leucovorin plus 5-fluorouracil 48-hour continuous infusion combined with cisplatin for advanced or recurrent nasopharygeal carcinoma(NPC)

Objective To assess the anti tumor efficacy and safety of high dose leucovorin plus 5 fluorouracil 48 hour continuous infusion combined with cisplatin in treating advanced or recurrent nasopharyngeal carcinoma(NPC).Methods Thirty patients with advanced or recurrent NPC were treated with high dose leucovorin plus 5 fluorouracil 48 hour continuous infusion combined with cisplatin.Results There were four complete response,eighteen partial response,seven no changes and one progressive disease in this series with a total response rate of 73.33%.The main side effects included nausea and vomiting,mucositis,bone marrow suppression and alopecia.All side effects were tolerable and mild.Conclusion High dose leucovorin plus 5 fluorouracil 48 hour continuous infusion combined with cisplatin may be a safe and effective therapy for patients with advanced or recurrent nasopharyngeal carcinoma(NPC).
