垂体后叶素在腹腔镜输卵管妊娠切开取胚术中的应用价值

目的:探讨腹腔镜输卵管妊娠切开取胚术中垂体后叶素的应用价值。方法:随机将2007年6月至2009年6月拟行腹腔镜输卵管妊娠切开取胚术的266例患者分为两组,术中注射垂体后叶素的160例为观察组,未注射的106例为对照组。比较两组手术时间、术中出血量、术后最高体温、保留输卵管成功率和持续性宫外孕发生率等。结果:观察组手术时间、术中出血量、持续性宫外孕发生率均明显少于对照组(P0.01);保留输卵管成功率明显高于对照组(P0.01)。两组术后最高体温差异无统计学意义(P0.05)。结论:腹腔镜输卵管切开取胚术中应用垂体后叶素能明显缩短手术时间,减少术中出血,降低持续性异位妊娠率,提高保留输卵管成功率,并且安全可靠,操作简单,适合基层医院和初学者应用。

Application value of posterior pituitary in laparoscopic salpingotomy and embryo removal for tubal pregnancy

Objective:To investigate the application value of posterior pituitary in laparoscopic salpingotomy and embryo removal for tubal pregnancy.Methods:The 266 patients who suffered tubal pregnancy and would undergo laparoscopic salpingotomy and embryo removal were randomly divided into 2 groups.160 patients who were injected posterior pituitary in operation were in observation group and 106 patients without injection belonged to control group.The operative time,intraoperative blood loss,postoperatively highest body temperature,success rate of reservation oviduct and incidence of persistent ectopic pregnancy between 2 groups were compared.Results:The operative time,intraoperative blood loss and incidence of persistent ectopic pregnancy in observation group were obviously less than those of control group(P<0.01),the success rate of reservation oviduct was higher than control group(P<0.01).There was no significant difference in postoperatively highest body temperature of 2 groups(P>0.05).Conclusions:Application of posterior pituitary in laparoscopic salpingotomy and embryo removal for tubal pregnancy can remarkably reduce operative time,intraoperative blood loss and incidence of persistent ectopic pregnancy,and improve the success rate of reservation oviduct.It is safe,reliable,easy,and appropriate for primary hospital and laparoscopic beginner.
