年龄对冠状动脉旁路移植术预后的影响

目的　探讨年龄对冠状动脉旁路移植术预后的影响。方法　对行冠状动脉旁路移植术的小于60岁、60～69岁和大于70岁3个年龄段的351例病人，在手术死亡率、并发症发生率、住院时间及住院费用方面进行对比分析。结果　3组病人的手术死亡率分别为1.5%、2.4%和7.8%(P=0.04)；早期并发症发生率分别为11.4%、14.9%和17.6%（P=0.49）；住院时间分别为(20.30±7.14)d、(20.30±6.76)d和(21.30±7.60)d，P=0.59；住院费用分别为(55105.0±26047.5)元、(64634.1±42893.2)元、(60307.8±22038.5)元，P=0.08。结论　随着心外科手术技术的进步，经过严格的手术适应证选择，70岁以下行冠状动脉旁路移植术的病人具有满意的临床结果，但70岁以上的病人应慎重。

Analysis of age factor affecting postoperative outcomes in coronary bypass surgery

Objective: To determine the age factor for prediction of postoperative outcomes in coronary artery bypass grafting. Methods: 351 patients who underwent coronary artery bypass grafting were divided into three groups (＜60 years, 60-69 years, ≥70 years). Analysis of outcomes including postoperative mortality, complication, length of hospital stay and costs was made. Results: The hospital mortality for three groups was 1.5%, 2.4% and 7.8% respectively (P=0.04). The incidence of complications was 11.4%, 14.9%, and 17.6% respectively (P=0.49). The mean hospital stay was (20.30±7.14) days, (20.30±6.76) days and (21.30±7.6) days, respectively (P=0.59). Conclusions: Coronary artery bypass grafting has an encouraging result in the patient. But selection should be cautious in the patients over 70.
