血管瘤和血管畸形的MRI表现与病理联系

摘    要:目的:探讨血管瘤与血管畸形的磁共振成像(MRI)表现和病理的联系。方法:对435例血管瘤和血管畸形MRI表现与病理进行对照。结果:205例血管瘤,T2WI:170例表现为较高信号,其病理表现为血管内皮细胞数量多,肥大,血管腔不明显;35例呈高信号,其病理表现为内皮细胞少,扁平,血管腔扩张,血管腔面积占镜下视野面积比例大,间质成分多。显示条状低信号分隔形成网格状改变的180例中,其病理学表现为纤维组织形成条状分隔瘤组织。230例血管畸形,T1WI、T2WI表现为均匀等信号15例,其病理镜下表现为细小薄壁血管组成。215例血管畸形T1WI、T2WI信号不均匀,其病理基础为血管粗大、变形,间质成分种类多,可见钙化,75例有出血,10例有血栓形成。结论:MRI可在一定程度上反映血管瘤和血管畸形的病理变化,血管瘤和血管畸形MRI的信号及形态改变与组织病理学呈现高度一致性。

Abstract:Objective: To observe relation of MRI appearances and pathological findings in hemangiomas and vascular malformations.Methods: MRI appearances of 435 cases hemangiomas and vascular malformations contrasted with their pathological findings.Results: Among 205 cases hemangiomas,170 cases showed increased signal on T2-weighted images.Their pathology features were more vascular endothelial cells,hypertrophy and less lumens of blood vessel.35 cases showed markedly increased signal,and their pathology features were few vascular endothelial cells,capillary lumens expanded,capillary enlarged lumen areas,and much mesenchyma.180 cases were separated by low signal network on T2-weighted images.Their pathology features were that tumor tissue was divided by fibrous tissue.Among 230 cases vascular malformations,15 cases showed homogeneous signal on T1 and T2weighted images,their pathology features were that blood vessels were thin.215 cases showed inhomogeneous signal on T1 and T2-weighted images,and their pathology features were large and distortion blood vessels,multiple mesenchyma,calcification in some cases,bleeding in 75 cases,and thrombin in 10 cases. Conclusion: MRI features can evaluate pathological change in hemangiomas and vascular malformations on some extent.The change of signal and configuration of MRI was closely correlated with component of pathology tissue.
