脊髓病变患者体感诱发电位检测及预后判断的可靠性

目的探讨体感诱发电位(SEP)在脊髓病变中的诊断价值。方法采用丹麦Keypoint肌电/诱发电位仪,对84例脊髓病变患者进行SEP检测。84例均行下肢胫后神经SEP(SEPt)检测,其中34例同时行上肢正中神经SEP(SEPm)检测。结果SEPt检测84例中82例异常(97.6%),SEPm检测34例中21例异常(61%),SEPt检测结果异常率明显高于SEPm。异常表现均以波幅降低为主,SEPt异常以波幅降低为主者占73.1%,SEPm异常以波幅降低为主者占90.5%。结论SEP反映了躯体感觉传导通路、脑干网状结构及大脑皮层的功能状态。脊髓病变SEPt检测敏感性明显大于SEPm,异常表现以电位波幅降低为主。因此,SEP不仅对脊髓病变的定位诊断有参考价值,而且对判断脊髓损伤程度、评估疗效及脊髓功能状态均有重要指导意义。

Somatosensory evoked potential per formed analysis on 84cases with spin al cord diseases

Objective To study the diagnostic values of som atosensory evoked p ot ential(SEP)in spinal cord diseases.Methods SEP was performed in 84patients with spi nal cord diseases.The tibial nerve o f the pelvic limb of SEP(SEPt )was performed in all of the 84pa-tients,and the median nerve of the th ora cic limb of SEP(SEPm)was performed in 34of the 84patients.Results The abn ormality of SEPt was found in 82ones of 84patients(97.6%),and the a bnormality of SEPm in 21ones of 34patients(61%).The abnormality rate o f SEPt was higher than one of SEPm.The abnormality of SEPt and SEPm mainly sho wed the decrease of amplitude.The rate o f the decrease of am-plitude in SEPt and SEPm was 73.1%and90.5%.Conclusion Somatosensory conduction way,t he r eticular of brainstem and the state o f cerebral cortex function was reflec ted by SEP.The sensitivity of SEPt wa s higher one of SEPm in the spinal cord d iseases.Therefore,not only the SEP had the reference value in top ical diagnos is of spinal cord diseases,but important guiding curative e ffect,and the stat e of spinal cord function.
