射线立体定向伽玛刀治疗听神经瘤后对颅神经功能影响的随诊分析

目的总结分析立体定向伽玛刀治疗听神经瘤后对肿瘤周围的面神经、听神经、三叉神经功能影响的随访结果。方法1997年7月～2005年11月本科采用立体定向伽玛刀治疗159例听神经瘤，获得随访137例，平均随诊37．2个月（6～116个月），肿瘤最大长轴直径6～33mm（平均25mm），最大横径7～36mm（平均16mm）。肿瘤边缘剂量10～186Gy（平均12Gy），40％～75％等剂量曲线，靶中心点1～12个（平均5个）。结果肿瘤总控制率94．16％，听力神经、面神经、三叉神经功能保留率分别为63．20％、77．37％、86．86％。结论立体定向伽玛刀治疗中小型听神经瘤具有良好的控制效果，对肿瘤周围的颅神经有较高的功能保留率。

Clinial follow-up and analysis of effect of postradiosurgical acoustic neuromas on peritumor cranial nerves

Objective To analyse the effect of postradiosurgical acoustic neuromas on facial nerve,acoustic nerve and trigeminal nerve.Methods Gamma knife radiosurgery(GKS) was performed in 159 patients with acoustic neuromas from 1997 to 2005 at our center.One hundred and thirty-seven cases were followed up with MR1 and other clinical examination.The median followup period was 37.2 months(6-116 months).Results The overall tumor control rate was 94.16%.The preservation rate of acoustic nerve,facial nerve and trigeminal nerve was 63.20%,77.37% and 86.86%,respectively.Conclusion GKS is confirmed to be an effective treatment for acoustic neuromas and plays an important role in preservation of peri-tumor cranial nerve.
