维生素D与初诊男性2型糖尿病患者并发血脂紊乱的关系研究

摘    要:目的 探讨初诊男性2型糖尿病患者血清25-羟维生素D[25(OH)D]水平及其与血脂水平的关系.方法 选取在医院内分泌科住院的105例初诊男性2型糖尿病患者,收集常规及生化指标检测,检测所有受试者血清25(OH)D水平.按照血清25(OH)D水平进行四分位法分组,对年龄、BMI、甲状旁腺激素(PTH)、糖化血红蛋白(HbA1c)、空腹血清葡萄糖(FBG)、血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血清钙(Ca)进行比较,并进行相关性分析.结果 维生素D正常11例(10.47%),25(OH)D<20ng/ml者62例(59.05%),20~30ng/ml者32例(30.48%).有血脂异常者85例(80.95%).Q1组TG和PTH水平均低于Q2、Q3、Q4组,HDL-C水平高于Q2、Q3、Q4组,差异有统计学意义(P〈0.05).血清25(OH)D水平与PTH、BMI、腹围、TG呈负相关,与HDL-C与呈正相关.结论 初诊男性2型糖尿病患者维生素D缺乏普遍存在,维生素D缺乏及继发性甲状旁腺激素升高与糖尿病相关血脂异常密切相关.

Abstract:Objective To explore the relationship of Serum 25-hydroxyvitamin D (25 (OH) D) and dyslipidemia in male patients with newly- diagnosed type 2 diabetes mellitus(T2DM). Methods 105 cases of patients with newly-diagnosed type 2 diabetes mellitus were selected as the object of study, the routine and biochemical indexes of which were collected and detected. All the subjects were detected the serum level of D 25 (OH). According to the level of 25 (OH)D, all subjects were divided into 4 groups. The data of age, BMI, parathyroid hormone (PTH), glycosylated hemoglobin (HbA1 c), fasting serum glucose (FBG), serum total cholesterol (TC), triglyceride (three TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) were compared and analyzed. Results Vitamin D was normal in 11 cases ( 10. 47 % ), 25 (OH) D 〈 20ng/ml ( in 62 cases, accounted for 59.05 % ), 20 ~ 30ng/ml ( in 32 cases, accounted for 30.48 % ). The patients with dyslipidemia were in 85 cases(80.95% ). The level of TG and PTH of Q1 group was lower than that of Q2, Q3 and Q4 group, and the level of HDL-C of Q1 group was higher than that of Q2, Q3 and Q4 group, and the differences were statistically significant ( P 〈 0.05 ). The serum 25 (OH) D level was negatively related with PTH, BMI, waistline, and positively related with HDL-C. Conclusion Vitamin D deficiency is common, and vitamin D insufficiency and secondary hyperparathy- roidism are associated with dyslipidemia in male patients with newly-diagnosed T2DM.
